Clinical Social Worker
Our clinical social worker will design a plan to
meet the needs of the client. Our social worker will
customize a care plan to include any doctor’s
recommendations for prescribed meal plans,
medication controls, and can even offer therapy
based upon the needs of the client. Our social
worker will contact you after our service has been
contracted to ensure that your needs are being met.

Personal Case Manager
The case manager will match the caregiver to the
individual and ensure the implementation of the
care plan. The case manager actively monitors
changing needs of the client during on-site visits.

Family Communication
If requested, the client’s family members will be
contacted by our case manager to report the condition and discuss the needs of the client. We provide frequent phone calls to discuss the activities performed and provide assessments of the moods, attitudes and behaviors of the client. Further, the caregiver will proactively look for deterioration of health and assess the overall well-being of the client.

Pre-Employment Screening
Neighborhood Home Care employees are pre-screened for honesty, integrity, and general aptitude. Prospective caregivers must pass a series of tests to determine if they have the abilities critical for success. Further, all employees’ backgrounds are checked through the FBI, Department of Justice, and Adult/Child Protective Services.

Trained Employees
Upon initial hiring, all employees of
Neighborhood Home Care are trained
by our social worker. The training
focuses on implementation of the care
plan, identifying the needs of the client,
communicating with the client, and
providing compassionate service.
Many of our caregivers have completed
CNA, CHHA, HHA and other training
programs. Additionally, our caregivers
are trained in CPR and first aid.

Our management staff holds advanced degrees in social work, nursing, business, and accounting. The many years of experience in various experience in various industries provides a foundation of professionalism, experience, and hard work that creates a dynamic company dedicated to serving the needs of the elderly.

Individual or Family Therapy
Through our clinical social worker, we offer both individual and family therapy based upon the needs of the client or family. The therapy is designed to assist the individuals as they cope with the problems and challenges of life and improve overall mental health.

Licensed Registered Nurse
If the needs of the individual require a registered nurse to check-in on your loved one, that service is available at your request. A registered nurse can make recommendations regarding specific in-home needs to make your loved one more independent and stable.

Home Cleaning Services
Neighborhood Home Care offers professional home cleaning and maid services that are designed to provide a deeper level of cleanliness. In addition to home cleaning and maid services, we also offer carpet cleaning, floor care (wood, vinyl and marble), power washing for the exterior of your home, and window cleaning. All services are available upon request.

Landscaping and Lawn Service
We can provide services for mowing lawns, edging the grass, removing weeds, raking leaves, gardening, fertilizing, trimming of trees, and repairs on the irrigation system.

Personal Supplies
Neighborhood Home Care can provide supplies for daily living such as incontinency pads, disposable gloves, soaker pads, peri-wipes, bath seats, walkers, blood pressure devices, and bedpans.

* Each care plan is developed, implemented and continually assessed by our clinical social worker. Our social worker also trains and supervises both the case managers and our caregivers. This approach allows our agency to maximize the care given to the individual.
